I am committed to improving the health outcomes of people focusing on what is most important to them. I am excited about the opportunity to contribute to the well-being of Veterans through personalized and impactful health programs. Throughout my career my responsibilities involved conducting interventions with patients through various modalities, including one-on-one, family and group sessions, both in outpatient and inpatient settings. As a Case Manager I worked with clients to identify their needs and utilize community resources while encouraging their autonomy. I have extensive experience in helping clients identify and pursue personal health and life goals. I work well with clients individually and in therapeutic and educational groups. In developing treatment plans, I utilized evidence-based behavioral interventions, customized to each patient’s needs and goals. These plans included personalized goals, action steps, and interventions. I am skilled at individualizing intervention strategies, helping clients utilize existing skills, and developing new ones for enhanced success. Regular ongoing sessions were scheduled to assess progress and navigate any obstacles or challenges that arose. I have personally experienced the positive results of compassionate self-care practices including mindfulness, positive interpersonal relationships, physical exercise and creative activities such as music, visual arts, etc., and use them to the best effect with my patients. I am eager to bring my skills, passion, and experience to the Cincinnati VA and contribute to the health and well-being of Veterans. As a former USMC reserve infantryman, I have a strong desire to work with and serve those who have served us all. I look forward to the opportunity to discuss how my background aligns with your organization's mission and goals. Thank you for considering my application. Peter K. Sloan
