- Provide direct support to clients at the Live By Facility and within the community.
- Develop, implement, and adjust goals and skills training based on client needs.
- Provide advocate services to clients; advocate for choice, needs and rights.
- Educate and support client interests.
- Participate in consumer-related meetings.
- Provides case management to clients in the form of; support coordination, documentation of progress on goals and skill building, communication with service providers, and conducting follow up as needed.
- Maintains accurate, complete, and confidential participant case files.
- Establishes a resource and referral network with relevant service providers.
- Assists in the preparation for the Council on Accreditation review.
- Uses a variety of positive, proactive behavior management techniques.
- Conducts self in professional manner in working with clients, social workers, and other professionals in the community.
