During my tenure at MD at Home, I worked primarily on preventing readmissions when patients transitioned from an acute care setting to home health services. In this capacity, I monitored and evaluated care outcomes and other performance indicators for over 40 patients.
I collaborated with caregivers to create an appropriate care profile for each patient and provided assistance to home-bound patients through telehealth services. I ensured timely review of patient charts and recommended corrective actions when necessary.
This experience helped me to expand my knowledge of homebound patient healthcare and patient chart analysis, while also improving my ability to collaborate effectively with clinicians.
