Ensured all patient demographics, insurance, and income information was accurately entered into both the Enrollment System as well as Vista.   • Monitored enrolled patient data base information on a regular basis and completed updates to health insurance data, income assessments for the Means Test and Pharmacy Co-pay Exemption programs as mandated by law. Remained diligent in performing audits and completing timely updates to any and all information provided by the veteran. Processed any and all incomplete encounters to ensure all patient data was correct. In doing so, any negative impact to the facility and veteran was greatly reduced.
  • Utilized scheduling software to ensure all new patients to the facility were scheduled with a Primary Care Provider within a timely manner. Was also proficient in creating Community Care referrals for veterans meeting the Mission Act Criteria.

Explained the process for applying as well the general structure for the program. Ensured any and all applications were submitted immediately, so as not to cause any delays in patient care.

Gathered all documentation needed per law and submitted the application to the privacy officer on behalf of the veteran. Ensured timely delivery of information, which meant veteran’s info could be updated to avoid any negative impact to health care.
  • Processed information related to Decedent Affairs. Gathered all information needed to process a patient’s death. Coordinated with family members, funeral homes, Primary Care Team and Decedent Affairs group to gather all necessary information pertaining to patient death. Worked with patient’s Primary Care Provider to have death certificates signed in a timely manner, which allowed the family to complete any and all post mortem services. Updated patient’s chart in CPRS to reflect patient’s death.
