1. Meet with the client within 24 hours of arrival, perform client intake, and explain the expectations of CCI R&P services.
2. Complete a client Self-Sufficiency Plan with the client, identifying strengths and areas of need or interest for the client.
3. Connect the client with the faith community, other community partners, the closest grocery store, and other basic needs.
4. Refer new clients to the Employment team for public transportation instruction.
5. Meet the client for a 30-day home visit. Visits must happen within the first 30 days of arrival.
6. Responsible for proper case noting and case documentation in the client’s case file.
7. Ensure the application of SS and FSSA for clients is complete.
8. Take clients to SS, FSSA, SVdP, and WIC appointments as necessary.
10. Other duties, as assigned.
