Telephone Triage -Triage incoming calls from patients by reviewing symptoms and advising on the best treatment plan to facilitate patient care. Counseled patients via telephone according to established guidelines and protocols. Relay triage information and facilitated communication between providers and patients.
Support PCMH metrics- Diabetic and hypertension education: Met with patient one on one for diabetic education. Monitored A1C and blood sugar readings to ensure compliance and relayed information to physicians. Assisted patients with diabetic supplies and how to use them correctly. Met with patients one on one to ensure they are taking blood pressure accurately, educated on proper technique and signs and symptoms to monitor.
Transitional Care Management- Monitered patients that were admitted to the hospital for hospital follow ups
and ensure that proper appointments and referrals were placed as well. Ensured that the patient had a clear
understanding of discharge instructions and follow up care.
Wound Care- Received verbal orders from physicians to perform wound care. Educated patients on how to treat wounds at home, provided education on signs of infection.
Trained newly hired RN/LVN including at different sites.
Verify medication administration for medical assistants.
Administer vaccines, other medications as needed.

• Nurse Only Visits- EKG’s, Blood Pressure checks, medication administer and teaching, administer PPDs and TB reads, COVID swabs, Strep swabs, throat cultures, influenza swabs. Other Miscellaneous tasks as directed by providers.
• Respiratory Clinic from 3/2020-11/2020- Assisted providers with COVID patients and treatments. Nurse Only visits for vital sign checks and Covid Swabs.

• Prepared patients for procedures, educated patients and family members on surgical procedures such as epidural steroid injections and lidocaine infusions.
• Monitored patients conditions including interpreting and tracking EKG readings, identifying irregular telemetry readings during lidocaine infusions and procedures and then updated providers on changes.
• Managed care for post procedure patients from initial recovery through discharge
• Managed Lidocaine infusion patients for chronic pain patients over a period of several hours.
• Called patients with pre procedure instructions and follow up calls.
