Telephone Triage -Triage incoming calls from patients by reviewing symptoms and advising on the best treatment plan to facilitate patient care. Counseled patients via telephone according to established guidelines and protocols. Relay triage information and facilitated communication between providers and patients.
Support PCMH metrics- Diabetic and hypertension education: Met with patient one on one for diabetic education. Monitored A1C and blood sugar readings to ensure compliance and relayed information to physicians. Assisted patients with diabetic supplies and how to use them correctly. Met with patients one on one to ensure they are taking blood pressure accurately, educated on proper technique and signs and symptoms to monitor.
Transitional Care Management- Monitered patients that were admitted to the hospital for hospital follow ups
and ensure that proper appointments and referrals were placed as well. Ensured that the patient had a clear
understanding of discharge instructions and follow up care.
Wound Care- Received verbal orders from physicians to perform wound care. Educated patients on how to treat wounds at home, provided education on signs of infection.
