Managed multiple residents in a 382 unit housing property, Provided information and resources for Paratransit, Doctor appointments, Money Management Community events, rental and utility assistance, SFHA recertification, intakes and coordination of adult actives.

Assessed barriers of individuals and families experiencing homelessness. Developing housing procurement.

this includes intake interview to determine client's needs goals and eligibility . Provide mediation and advocacy with landlords on client's behalf. Create and maintain consistent communication channels, both verbal and written, between several parties(i.e. tenant, landlord, referral source, collaborating agencies, debtors and creditors). Serve as an ongoing liaison between property managers and participants a well as between participants and neighbors. Provide information and referral assistance regarding available support from appropriate social service agencies and/or community programs

Case Management for 30 residents including coordinating services with external agencies, facilitating and supporting regular tenant meetings, maintaining records and files, which includes documentation of all services per agency standards. Activity Coordination: Plan direct on and off-site social activities, workshops and educational programs for residents in several supportive housing sites

Case Management for formerly homeless families housed on Treasure Island NAS. Intakes and information and referral for services. Facilitation and supporting regular tenant meetings, maintaining records and files, preparing and presenting reports. Plan and directed social activities
