  • Provides, coordinates, teaches, or supervises nursing care for a group of patients, usually in the setting of the patients' home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and caregivers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals.
