I was employed at a Government hospital, in the rural town of Nanjangud, Karnataka, India, as an Intensive Care Unit Medical Officer (ICU MO) for one year and was involved in the management and provision of treatment to cases admitted to the ICU, in collaboration with the ICU in-charge of our hospital. My job as the ICU MO also involved attending virtual ICU rounds, interacting with senior consultants at a tertiary care, multi-specialty government hospital, for discussion of patients admitted to our hospital as per protocols by the Government of Karnataka for the 10-bed ICU program for all rural government general hospitals (GHs) in the state. Some of the cases were:

  • Congestive heart failure, Hyperosmolar Hyperglycemic State, Acute exacerbation of asthma, Acute exacerbation of COPD, Lower respiratory tract infections, Trauma cases: epidural hematoma on NCCT head managed conservatively, Liver diseases such as alcoholic hepatitis, chronic liver disease with and without decompensation, and alcohol withdrawal cases/alcohol dependence, Toxicology: Paracetamol, organophosphorous, and aluminium phosphide poisoning, Snake bite, Acute gastroenteritis with severe dehydration and hypotensive shock, and Iron deficiency anemia requiring PRBC transfusion.

I was assigned duties in the ER as In-charge in accordance to duty roasters with provision of treatment to emergency cases such as:

  • STEMI cases, NSTEMI cases, Acute exacerbation of COPD, HHS, Hypoglycemia, and Seizures

I was also assigned duties for the provision of treatment to OPD-based routine cases as the Outpatient Department Care In-charge, in accordance with duties as per rosters (see attachment for details), such as:

  • Administration of medications for Type 2 Diabetes Mellitus, hypertension, thyroid disorders, medications for upper respiratory tract infections, urinary tract infections, acute gastroenteritis, anemia, administration of ARV (anti-rabies vaccine) for Category 2 dog bites, and immunoglobulin plus vaccine given for Category 3 dog bites.
