As the PMHNP, I am providing psychiatric care to incarcerated individuals with a combination of psych evaluations and follow ups for mental health care and medication management. In addition to the outpatient inmates being seen, I am responsible for rounding on individuals placed on SHOS (self harm observation status) or MHOS (mental health observation status) who may be a potential threat to themselves or others and determining admission or discharge from the compound infirmary. On average, I am seeing 15 patients in an 8 hour day.

-Enhanced patient outcomes by developing comprehensive treatment plans and medication management tailored to individual needs.

-Improved care continuity by effectively collaborating with interdisciplinary team members and maintaining clear communication.

Responsible for Psychiatric and Medical evaluation, treatment planning with designated treatment team, care management and discharge planning for involuntary court ordered patients on a 29 bed adult unit. The population predominately consisted of the severely mentally ill including individuals with schizophrenia, bipolar disorder, and major depressive disorder.

-Documented observations, interventions and concerns in patient charts and electronic systems, sustaining continuum of care from admission through to discharge

-Recorded comprehensive patient histories and coordinated treatment plans with multi-disciplinary team members.

-Provide direct patient care with treatment, stabilization, and education on disease process and management and lifestyle options

-Administer prescribed medications via oral and/or intramuscular injections and monitor responses.

-Educated patients and families on medical-surgical procedures, therapeutic regimens, and pain management techniques

-Started intravenous fluids, administered medications, and documented patient charts by recording food and fluid output and intake

-Managed caseload of 6 patients simultaneously by providing bedside care, treatments, IV therapy, and wound care

-Managed patients' pain by implementing pharmacological and non-pharmacological pain management interventions.
