Improve patient outcomes with compassionate, in-home care to include physical assessments, medication management, wound care, catheter management, lab draws, collaboration with interdisciplinary team, development of personalized care plans, accurate and timely documentation of care per CMS standards.

• Educate patients and families on disease management, medication safety, and at-home healthcare strategies to promote selfcare.
• Ensure complete planning for patients and coordinate referrals and outside services received or needed by patients. Develop a plan of care with care team according to clinical practice guidelines with patient, family, caregiver(s) and significant other(s). Identifies resources to integrate additional clinical practice guidelines to support continuity of care.

• Direct patient care using the nursing process and critical thinking; collaborate with interdisciplinary professionals, patients, and families; prioritization of care; clinical documentation, patient education and advocacy.
• Develop and maintain patient registries based on disease, health condition and/or health prevention initiatives.
• Collect subjective and objective health status data from multiple sources, which includes patient, family members, team members and documented records. Analyzes health status data in terms of the unique individual and/or disease related situation.
• Care management involving patient assessment, initiating and maintaining individualized multi-disciplinary plan or care involving patients and their families to give optimal care.
• Assist with triage of patients, including triaging telephone calls and walk-in patients. Assess patient needs, asks appropriate questions, and use clinical judgment, knowledge and experience in directing patients.
• Precept and train new nurses in proper techniques, care standards, operational procedures, and safety protocols.
