Triaging patient utilizing the nursing process to provide care advice to patients/caregivers by conducting focused assessments. Functions independently utilizing nursing processes. Ability to prioritize and perform effectively under stress. Ensure that the client’s plan of care is individualized and appropriate, executed as written and reassessed by the appropriate health care professional when there is a significant health status change in the client’s condition, at the physician’s request and after hospital discharge. Hold weekly Case Conferences to discuss client issues. Determine if clients that are planned for discharge meet the requirements for Management and Evaluation or any other skilled service. Evaluate performance relative to job goals/requirements; coach staff and recommends in-service education programs and ensure adherence to internal policies/standards. Promote quality and participate in care coordination to ensure proper communication between caregivers, patients, referral sources and payers. Have oversight of clinical integrity of appropriate documentation, quality of care provided, visits utilization, appropriate contacts with physicians, adherence to the care plan and evidence of care coordination between disciplines. Provide oversight of communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning and ensure compliance through review of documentation and care coordination activities. Be responsible for continual survey readiness. Assist Director with managing timeliness and responding to ADRs and other external requests. Be competent with the State and Federal Survey process and ensure that the agency as a whole and all the agency staff are prepared for on-site surveys and other external audits. Function in the capacity of staff nurse when necessary. Participate in community awareness and education programs. Take administrative on-call as a support to the on-call nurse and to provide software management related to processing intakes, reviewing evaluation packets, and scheduling, moving, reassigning visits to ensure the smooth function of the Agency during off hours.
Manage the policies and procedures as related to the care that is provided to the patients admitted for home health services. Responsible for the application of guidelines and standards in accordance with current practices Patient care activities include: prioritizing work by preparing daily visitation lists. Have knowledge of and understand current Medicare guidelines for the clinical aspects of the department. Promote teamwork with clinicians and all members of the inter-disciplinary team. Assist in departmental and community in-service/education when assigned, actively participates in Performance Improvement, assigned meetings and lead the weekly case conference meetings to discuss patient care needs as well as address staff concerns. Uphold professional standards of performance and conduct. Provides direct patient care when needed. Have knowledge base and understanding of Medicare and HIPPA guidelines. Attend in-services and training as required by the Texas Department of Human Services to maintain eligibility in the alternate administrator role. Perform visits as needed. Participate in administrative on call rotation.
IN THE ABSENCE OF THE ADMDINISTRATOR:
Organize and direct the department’s ongoing functions in compliance with all applicable state and federal rules/regulations. Assisting in meeting and exceeding the expectations of COPs, TACH and agency policies. Work closely with HR department relating to recruitment, hiring, orientation and ongoing training of the office and field staff. Provide planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.

Organizing and facilitating meetings with security managers to ensure infections-control policy compliance.
