I transport patients from a hospital to a psych hospital or from one psych to another psych unit. And also transport patients in a wheelchair from a facility such as a Hospital, Nursing Home doctors office or Dialysis etc. I always check with the nurse if at a facility to make sure that the patient is ready to be transported and the patient didn't to be given any need medications or tests. Also gather any paperwork or medications. I introduce myself to the patient gathering their belongings including cell phone chargers. Make sure that the patient is safely secured with their seat belt and safety straps securing the wheelchair. I also transport patients from their home to a doctors appointment or medical appointment that they need to go to.

• Checked patient vitals such as temperature, blood pressure, and blood sugar levels. Also checked on my patients and if I noticed any changes in a patients condition I would notify the nurse immediately . I always talk with my patients making sure that they were comfortable. They often committed that they knew that I cared about them and not just doing my job.
