  • Reviews completed Telemove Tech Education notes, sighs, and mails welcome packet and other materials as requested. Documents when they were mailed in CPRS by writing an addendum to the tech education note.
  • Orders equipment from the Remote Order Entry System (ROES) within 24 hours of receiving aa chart alert based on what the RD has indicated should be ordered. Documents in CPRS when this was done by writing an addendum to the Tech Education note.
  • Integrates Veteran in VISTA by signing up under "home" (home telehealth menu) >"SU" (pt sigh-up/activation) so the vendor sites can connect with CPRS medical records to generate reports. Immediately after ordering equipment.
  • Submits a LEAF request to enroll veteran into the Telemove Secure messaging clinic. Documents completion on the daily telemove assignment log using Excel spreadsheets.
  • Once equipment has been received, monitors vendor site to ensure first check in is complete with 2 days of account activation. Contacts Veteran to help with completing first daily check if it appears veteran has not attempted to log in. Documents this as a telemove tech education note.
  • Reviews data daily for veterans who appear to have 7 and 14 consecutive days of nonparticipation/nonresponse days. Contacts veterans on day 7 and day 14 regarding non response. Prints off and mails appropriate communication after the call. Historically documents call in CPRS and attaches a copy of the letter mailed to veteran in CPRS using the LS progress note title. Identifies RD as an additional signer to the note.
  • Trouble shoots issues with equipment with veterans reporting problems and documents in CPRS using the tech education note.
  • Reviews Telemove Discharge Note. Prints off and mail identified discharge correspondence to veteran. Unenrolls equipment by discontinuing Service Enrollment in ROES. Inactivates veteran participation in VISTA. Documents when this was done in CPRS by writing an addendum to the Telemove Discharge note.
  • Mail books, MOVE! Program completion material, ect. to specified veterans requested by MOVE! Clinic providers.
  • Print off shipping labels/mail cases MOVE books and other MOVE program supplies to identified clinic locations.
  • Enter paper copy V signals data from returned surveys using a phone with a QR code reader. Returns survey to MOVE coordinator with date data was entered.
  • Print and assemble HTK packets for mailing/mail packets to specified veterans.
  • Yearly assembles materials for educational packets for Health Equity project for mail out. Address packets and mail out by specified date as directed by MOVE! Program Coordinator. Track returned packets on the mailout spreadsheet using Excel.
  • Yearly calls to Veterans identified in the Health Equity Cohort about interest in various programs offered, assemble list of veterans who identified they were interested in enrolling into a MOVE or HTK class.
  • Stuff envelopes with MOVE questionnaires and clinic brochures. Address/mail material to veterans identified by RD's needing to have MOVE informational materials.
  • Print and assemble materials required for MOVE orientation and mail out to all veterans identified on the Move Orientation Roster.
  • Enter data for returned MOVE questionnaires into the MOVE 11 questionnaire site. Once data has been entered, copy the retrieval code that is generated, and annotate the spreadsheet with the veteran's name, last 4, and retrieval code.
  • Scan and secure email all returned eating disorder screen reports to identified RD for review and entry into CPRS. Updates MOVE orientation roster to reflect ED screen and MOVE 11 were returned.
  • Utilizes CPRS, ROES, VISTA, LEAF, ROES, Microsoft Teams, Excel Spreadsheet, MS WORD, and TMS.
  • Adheres to all safety and sanitation standards that promotes high reliability organization principles and ensures public trust in VA by being consistently accountable and transparent.
