Help manage and maintain PointClickCare database within PACS, handling system configuration, user support, and data security compliance. Assist users to optimize clinical workflows, improve data accuracy, and enhance healthcare delivery by effectively integrating EHR systems across the organization. Conduct facility on-site visits for newly acquired teams and provide continuous re-education for current teams in the organization. Host webinars for teams regarding clinical modules.

• Responsible for assessment, treatment planning, intervention, monitoring, evaluation and documentation of patient's plan of care, overseeing weekly rehab meetings with physician and RN liaison, chart review, communicate weekly rehab progress to health plans, coordinate discharge needs for PT/OT and any medical equipment
• Provide and coordinate caregiver training for upcoming discharges
• Daily admission chart review, attending IDT weekly meetings and inservices
• Provide support and reeducation to clinical staff, assist in carrying out daily tasks
• Provide clinical resources to all staff in overseeing each patient's overall health, plan of care and safety
