• Responsible for participating in the screening and interviewing process of prospective clients.
• Develops and maintains comprehensive service plans with participants
• Assists in preparing monthly statistical reports as necessary.
• Records and maintains pertinent information, according to the program guidelines, regarding resident needs, concerns, and presents this information to the Program Director.
• Responsible for the care and use of materials, furnishings and cleanliness of the facility. Informs Director whenever replacement or repairs are needed.
• Provides physical care and comfort measures to the dying and supports their family and loved ones.
• Supports mothers in their role and tasks of parenting.
• Required to write Incident Reports when necessary and other reports as indicated by supervisor.
• Responsible for the facility (including being on call)
• Develop and maintain agency partner relationship and collaboration.
•Responsible for working with clients to identify personal strengths and resources and encourages the client to take charge of their recovery.
•Provides extensive and intensive case management and follow-up services.
•Provides crisis intervention to individuals and families facing emergencies around food, shelter, medical, re-construction and other needs as presented by client.
•Responsible for assisting clients in accessing community resources to address unmet needs not met by personal resources while still maintaining client dignity and confidentiality. The client’s successful recovery is dependent upon the continued partnership with case management.
