Ryan Hackenberg

• Work with patient's and their families to coordinate and establish safe and effective discharge planning back to their home or community setting.
• Work closely with Multi Disciplinary Teams to help identify and eliminate discharge barriers and develop effective discharge plans to meet patient's bio-psycho-social needs.
• Develop and maintain professional relationships with hospital professionals and community entities (Skilled Nursing Facilities, Rehabilitation Units, Home Health Agencies, Department of Aging, etc.)
• Work closely with Utilization Review Department to ensure of timely and medically appropriate discharges.

Caseworker 2

• Worked closely with individual families and their children to identify areas of abuse or neglect and to develop treatment plans to try to eliminate child welfare concerns.
• Work with numerous community entities to ensure for the safety of children in their homes or in their out of home placements.
• Take phone calls from the community and process into formal referrals to be assessed.

Worked in the General Protective Unit to provide case management services to keep children safely in their home or to develop plans to help get children returned to their parent's care.
