Process all new Conservatee’s appointed to the LPS Conservatorship in Avatar and our Database, which includes, opening episodes, assign clients to caseloads, input diagnosis update, submit client referrals, and update client alerts. Schedule appointments for clients to be evaluated by our Doctors for the Renewal or their LPS Conservatorship. Notify all facilities with upcoming Renewal Evaluations and Court Appointments and request Medical Documents needed for the Doctors to Evaluate the Clients in a timely fashion. Draft Blank Declarations for all Renewal appointments and send to the Doctors. Process Declarations submitted to us by the Doctor and send them to the County Counsel at the Courts. Process Letters, Orders, and Minute Orders and submit them to Medical Records. Create, update, and submit a spread sheet with Client Info and facility contact information for the Court Hearing to the County Counsel on a weekly basis.
