- Worked with geriatric population diagnosed with(CHF, Dementia, CVA, CRF, ….etc.
- Provide direct client care under supervision, progressing to manage a full caseload. Key duties include evaluating residents, creating treatment plans for ADLs/IADLs, leading therapeutic groups, conducting discharge planning, and documenting, with a focus on functional independence and safety.
- Administering standardized assessments such as SLUMS, Allen Cognitive Level Screen, and functional ADL checklists to determine baseline abilities.
- Intervention Planning & Execution: Developing client-centered, occupation-based interventions for conditions like stroke, ortho injuries and dementia.
- Documentation: Completing daily notes, progress reports, and discharge summaries in the electronic medical record (EMR), usually in compliance with Medicare guidelines.
- Participating in rehab meetings and collaborating with Physical Therapists (PT), Speech-Language Pathologists (SLP), and nurses to facilitate discharge planning.
- Professional Development: Assuming full caseload responsibilities, demonstrating ethical decision-making, and utilizing supervision for reflection and growth.
* implement therapeutic exercises to improve strength, coordination, balance, and range of motion, tailored to ADL performance. Common interventions include upper/lower extremity strengthening with Therabands, functional mobility training, seated exercises, and cognitive-based activities. These activities directly support improving independence in dressing, bathing, and grooming.
As well using Therabands for seated rows, chest presses, or shoulder exercises.
- Fine Motor Coordination: Sorting items, using reachers, folding laundry, or placing cones to improve dexterity.
- Balance & Core: Seated or standing, such as weight shifting, reaching for objects outside of base support, or using exercise balls.
- Functional Mobility: Practicing bed mobility (rolling, supine-to-sit), functional transfers (bed to chair, toilet), and, if appropriate, gait training.
- Cognitive/Sensory: Simple crafts, sorting, or using puzzles to engage in visual-perceptual skills to address memory, sequencing, and problem-solving.
