• Performs detailed Initial Clinical assessment of Oncology medication including educating and counseling patient on therapeutic use and directions of therapy, potential side effect, warnings and precautions, storage and other pertinent counseling information.
• Review chart notes, labs, scans and other materials from EPIC and communicate with patient's healthcare provider or teams regarding patient specific care concerns and findings.
• Collaborates with oncology team, and participates with interdisciplinary team activities to provide optimal patient centered chemotherapy regimen and treatment plan.
• Collaborates with Oncology/Hematology providers, inpatient oncology pharmacy team, as well as infusion center team in coming up with optimal chemotherapy treatment plan regimen.
• Participates in interdisciplinary team activities to develop and implement clinical care plans and disease state management programs involving medication therapy, collaborative practice, treatment protocols and medication error reduction.
• Works with providers team in submitting and helping to obtain approval of PA's including appeals of patient's specialty medication.
