As an LVN -MDS Coordinator for the last 14 years, I have worked with both Point Click Care and Matrix software. I performed resident assessments and gathered resident data and hospital records for the completion and accuracy of each individualized Minimum Data Set. Additional duties include implementing comprehensive care plans for each resident, updating it with each new assessments scheduling, communication with the nursing staff in relation to correct and complete documentation. Other duties include assisting with maintaining accurate medical information, its privacy and timelines related to completion of individual LTCMI as it correlates with reimbursement.
