Coordinate and manage care for high-risk obstetric patients across the antepartum, intrapartum, and postpartum continuum

Perform concurrent clinical review to ensure medical necessity, appropriate level of care, and alignment with evidence-based guidelines

Collaborate with OB, MFM, anesthesia, neonatology, social work, and interdisciplinary teams to develop and execute individualized care plans

Facilitate timely care transitions, discharge planning, and post-acute follow-up for complex maternal and neonatal cases

Identify barriers to care and implement interventions to reduce length of stay and prevent readmissions

Support utilization management through accurate documentation, payer communication, and authorization processes

Monitor patient progress and escalate variances related to complications, delays, or deviations from plan of care

Advocate for patients and families while balancing clinical quality, safety, and resource stewardship

Contribute to quality improvement initiatives focused on maternal outcomes, care coordination, and regulatory compliance
