Will be responsible for meeting compliance requirements for stat regulations governing senior housing and the certification process. Responsible for directing and implementing total case management for all housing clients on a regular basis. Includes personal care management, medication administration, care planning, scheduling physicians visits, setting up appointments, and working with clients, families hone health services, county and state offices to assure client needs are met. Responsible to oversee and conduct specified training for clinical staff to meet regulatory requirements on an ongoing basis. Responsible for coordinating with nursing leadership to identify needs for additional staff educational topics through the performance improvement process. General knowledge of instructional and communication skill, and group dynamics is required.
