Utilizing the check list: the UMC speaks to patient(s) and explains follow up appointment with time and date, makes sure that the patient agrees with the time and date of appointment. If patient declines and wants a different date or time, the UMC must call the follow up doctor’s office and change the date and time if necessary
Inform the clinical team that the appointment was changed and the reason. If the patient declines assistance with scheduling the follow up appointment, the UMC will notify the Team.
Speak to patient about transportation to follow up appointments if needed.
Call the insurance company to check transportation benefits for discharge purposes and for post discharge follow up appointments.
If transportation is not covered, inform the Social Worker
