Direct Patient Care in the home with multiple Chronic diagnoses. Follow Plan of Care per MD, Administer medications, complete clinical notes in a timely manner, vital sign, Respiratory Management and treatment. Infection control and management.

Intermittent Home visits providing skilled assessment, observation and disease management

Recorded patients' medical history, vital statistic and test results in medical records

One-on-one patient teaching for hypertension, diabetes and insulin management
