Create individualized plan of care for patients. Work with a team of PT, RN, and HHA to provide patient care. Administer injectable and PO medication as needed. Assess, clean and dress patient wounds as needed. Perform head to toe physical assessment and vital signs monitoring. Monitor patients for changes in their health and the progress of their recovery. Educate patients on their disease process, management of conditions, and lifestyle modifications. Provide covid-19 and influenza vaccinations to homebound patients. Perform admission and discharge assessments for patients under my care.
