Assess office and in center patients for home therapy candidacy, educate patients and family members on dialysis modality options which include transplant, peritoneal dialysis, home hemodialysis using NxStage as well as in center dialysis. Training of care givers and patients on all aspects of home therapy modalities. Performing venipuncture for blood work, medication review and administration, monthly nursing assessment, interdisciplinary team, care planning, rounding with MDs, coordination of appointments with surgical offices, review of QAI with DO.

Floating to multiple locations while performing hemodialysis and peritoneal dialysis treatments from start to finish, administer medications, monitor response to medications, follow lab values, communicate with multidisciplinary team to ensure quality care, educate patients and families in regards to hemodialyis and peritoneal dialysis.

Perform hemodialysis and peritoneal dialysis on both stable and critically ill patients in the acute care settings at 4 locations as a float RN, administer medications while monitoring response, evaluating daily labs and making adjustments as needed based on results, coordination of care between healthcare teams to ensure quality of care maintained, educating patients and families of effective treatment options as well as how to improve lifestyle choices for the best long term health outcomes

In addition to previously stated dialysis experience My position included supervising the clinic, guiding and training employees, ensuring staff maintains safe, quality care. Delegate tasks as necessary. Obtaining labs, medication administration, reviewing the results and outcomes with patients and doctors, round with doctors and suggest appropriate treatment changes as needed, educating patients and their families, coordination of care with outside physicians, facilities, and patients families as well as evaluating patient care and staff and alert manager if need be.
