    Served as pharmacist representative for Visiting Nurses Association with the Johns Hopkins Home Infusion Collaborative to eliminate central line associated bloodstream infections. Collaborated with nursing teams to begin educating nurses to use software to track line infections. Goals to track and identify patterns that predispose patient to line infections. Adjust equipment dispensed /education provided as needed

    Developed dispensing plan and protocol for use of 70% Ethanol Lock flushes in home care setting for patients who required long term IV access. This was a new initiative to further reduce catheter associated line infections.

    Collaborated with Kaleida IT to develop an electronic board to track and monitor patients over 18-24 month period. Served as pharmacist advisor closely with IT providers during regular meetings to develop software to streamline pharmaceutical care provided based on line of therapy, nursing team and geographic location. E-board vastly improved efficiency in providing excellent and timely care to our patients.

    Created template to electronically document licensed professionals responsible for programming infusion pumps dispensed to provide individualized infusion therapy.

    Assisted auditors with VNA to properly document diagnosis codes and culture data to ensure proper and timely reimbursement from insurance companies. Developed physician order note template to give auditors quick access to diagnosis codes, cultures and clinical data.

    Created an intake referral sheet for VNA liaison nurses to streamline information / orders needed to provided seamless continuity of care from inpatient facilities to the home care setting

    Conducted biannual aseptic /sterility testing of all pharmacists and pharmacy technicians. Maintained records for aseptic technique competencies for all compounding employees. Provided education and training on compounding of sterile products when needed. Re-tested as needed. Conducted these tests and training of new employees.

    Presented to liaison team and managers in pharmacy prescription requirements and general regulations governing the practice of pharmacy in NYS.

    Served as a pharmacy preceptor with the State University at Buffalo School of Pharmacy and Pharmaceutical Sciences providing students with a unique and valuable experience in home infusion. Precepted students on 8 week and introductory rotations. Frequently precepted students / interns assigned to our pharmacy director.
