  • Creates formal assessment reports that include a thorough description of the individual’s developmental history, behavioral history, and current needs based on interviews with the individuals and their family members as well as reviewing and analyzing documentation; to include psychological assessments and Individualized Education Program plans
  • Analyze’ client’s history, current living situation and overall health and safety to create and complete Health Risk Screening Charts for vulnerable adults to flag health and safety concerns
  • Through the analysis of facts and application of the legal requirements, determines if a mandated report to Adult Protective Services and Child Protective Services is needed
  • If so, creates and submits the reports
  • Conducts interviews with individuals applying for supports and services with the Maryland Department of Health and Developmental Disabilities Administration (DDA) to determine eligibility and appropriate placement
  • Analyzes program outcome reports and data to determine overall success of the team regarding client retention and profitability
  • Coordinates with members of client’s families to schedule and conduct meetings concerning the client’s treatment plans
  • Develops and conducts training programs for comprehensive assessment coordinators
  • Provides outreach and guidance to family members regarding the process of applying for DDA services, timeline, and resources available
