Follow all Title 22 regulations. ▪ Interview and screen all potential participants to determine their appropriateness for participation in the adult day/day health care program. ▪ Conduct assessments of each participant. ▪ Prepare a comprehensive care plan for each participant per Title 22 regulations and agency policies. ▪ Input accurate and detailed progress notes into the organization’s electronic health record according to the care plan to ensure comprehensive documentation. ▪ Maintain all social services components of the participant health records. ▪ Ensure all participant health records are up to date for all disciplines each month. ▪ Identify and provide referrals to available community resources for the participant and the caregiver. ▪ Serve as a liaison with community agencies that provide services to the participants, such as, but not limited to IHSS, Home Health Agencies, Case Management, Transportation and Hospice. ▪ Provide effective and inspiring leadership by being actively involved with the participants, families and staff. ▪ Lead, coach, develop and retain the center team. ▪ Oversee day-to-day operation of the center and program staff. ▪ Conduct daily stand-up meetings at the start of each workday. ▪ Ensure ongoing staff training per schedule and per regulations. ▪ Abide by and enforce organization policy and procedures. ▪ Serve as a member of the Multi-Disciplinary Team ▪ Serve as a member of the Utilization Review Committee. ▪ Attend monthly Leadership Team Meetings. ▪ Represent the organization at professional/educational meetings, speaking engagements and agency events, as needed. ▪ Responsible for oversight of all staffing including new hires, employee coaching and disciplinary action.
