Providing patients with wound care and educating caregivers on that treatment, wound vacs and pleurx drains. Accessing Mediport, PICC line and IV access, dressing changes for those lines and labs as ordered from the access device. Treating patients that are high risk for rehospitalizations, educating on Diabetes, Congestive Heart Failure, COPD and multiple other diagnosis. Educated patients on each medication they are taking and continue to review medications. Communication with MD's, Nurse Practioners and other staff memebers that are necessary to relay patient information and new orders. Conducted weekly interdisciplinary team meetings to review case management activities for assigned caseloads. Communication with other interdisciplinary team members regarding patient issues and specific needs. On Call time as requested by management. Nurse preceptor for multiple nurses at Caretenders.

Patient admissions and discharges, medication administration and medication education, IV medication administration and woun cdare. Communication with MD's, staff members and management staff.

Home visits to assess for placement at the facility, education to all staff, managing scheduling of nurses and aids. Communication with management staff,nursing staff, residents, family's and MD's. Step up when nurses and unavailable, assist with meal passes, medication administration and ADL's as needed.
