  • Provide services to consumers in accordance with the principles and practices of the ACT Fidelity Model. a. Lead the process of developing, updating, and guiding implementation of consumers’ Individual Recovery Plan (IRP), in collaboration with consumer, Team Leader and other significant parties of the ACT team. b. Coordinate treatment planning and discharge meetings at area hospitals to ensure a smooth transition to the community. c. Develop and maintain a professional and clinical relationship with area hospital staff to ensure a smooth transition for agency consumers to and from the hospital. d. Educate and assist consumers with learning or re-learning activities of daily living (ADL) skills necessary to improve the quality of life including acquisition of consumer health care benefits. e. Update Consumer’s IRP, Functional Psychosocial Assessment (FPSA), Level of Care Utilization System (LOCUS), Global Appraisal of Individual Needs (GAINS) and Crisis Plan every 180 days. f. Provide an average of 4.25 hours of services per day to ACT consumers. g. Establish constructive relationships with consumers to promote acquisition of rehabilitative goals. h. Maintain adherence to all policies and procedures consistent with Dept. of Mental health (DMH) and the District of Columbia board of social work.
  • Participate in treatment team meetings and conduct follows ups as indicated in the following areas: a. Crisis assessment and intervention b. Symptom assessment, management and individual supportive therapy c. Assistance in the treatment of substance abuse for consumers with cooccurring addictive disorders d. Psychosocial rehabilitation and skill development e. Recommend and sometimes make decisions about the best course of action for a particular service user f. Provide support services to ACT team and consumers g. Develop IRP and other care plans as indicated h. Conduct needs and risk assessment i. Provide resolution and referrals services for consumers j. Conduct psychosocial assessments using the Functional Psychosocial Assessment (FPSA) every 180 days for consumers served by the ACT team k. Education, support and consultation to consumers’ families and/or their support system, which is directed exclusively to the well-being and benefit of the consumer.
  • 3. Develop and implement quality improvement (QI) activities for individual clinical staff members and the clinical aspects of the ACT team. Attend QI meetings as appropriate;
  • 4. Ensure that all IRP’s, FPSA’s, and other pertinent paperwork is submitted on time.
