Schedule and complete assigned annual visits with IRIS members and guardians/primary caregivers/contacts, to review LTCF and obtain current health and support needs to determine functional eligibility for program, as outlined in the Wisconsin LTCFS Instructions.
This includes annual rescreen and change in condition screens, obtaining collateral contact with any IRIS paid and informal supports, and verifying diagnosis with physicians and/or Social Security Administration.
