  • I am currently still employed by this family, and they are aware that I am looking for new employment due to my upcoming relocation to Washington.

  • As an ARMHS (Adult Rehabilitative Mental Health Services) worker and Mental Health Practitioner my role was to fill out and maintain client-related paperwork, including federal and state-mandated forms, client diagnostic records, treatment plans, and progress notes while also maintaining confidentiality of these records
  • I regularly collaborated with mental health professionals and other staff members to perform assessments and develop and implement treatment plans based on clinical experience and knowledge.
  • I gathered information about community mental health needs and resources to be used in conjunction with therapy, guided clients on utilizing these as well as the skills and strategies they were working on in therapy, and continuously assessed clients for risk of mental health crises
  • Lastly, and my least regularly occurring role in this job was to organize and lead structured group programs of counseling, work study, recreation, and social activities as my typical role was in one-on-one interactions with clients

  • In my time at MAC, I provided one-on-one and group therapy sessions for clients, evaluated client responses to therapy and adjusted treatment plans according to their progress, reviewed client medical and mental health histories to pinpoint ideal treatment strategies, and in doing so helped clients develop plans to achieve personal, social, and vocational goals.

Private Long Term Hospice Care

Job overview

  • In my three years as a hospice care giver, I provided care for three different clients, each for about a year.
  • This was primarily a part-time position, but as each client's needs inevitably changed, I moved into a full-time position
  • I worked as a private contractor with an RN who provided medical services.
  • My role was to provide comfort and companionship to clients in their last year of life, as well as to assist where legally allowed with medical care.
  • Some of the services I provided were to communicate with family members about the progress of the client's condition and mental status, cleaning, sanitizing, and organizing the home in a way that was most functional for the client, turned and repositioned bed ridden clients, observing and recording client responses, reactions, and changes to medications, collaborating with other professionals to evaluate client's medical and physical condition and to assess client needs, and applying clean dressings to wounds and medical devices (colostomy bag, implanted port, etc)
  • Oversaw implementation and ongoing assessment of patient care plan.
  • The hardest, and yet, most rewarding part of this job always came when they were in their last weeks of life. During this time I would transition to working full time with them to be sure there was someone there with them at all times.
  • I was responsible for being present with each client to provide comfort and support during their last moments of life, as well as to contact their families afterward or in some cases provide emotional support for families during the transition in cases where they were present.
  • It was this that made me see the importance of human connection and having a caring and compassionate person there during the most difficult times
  • Though I have lost contact over the years with most of the families I worked with in this job, I have stayed in touch with Theresa Welna, the RN that I worked with.
