Key responsibilities or duties are promoting an environment in which elderly persons and persons with disabilities can live independently and remain in their communities.
I help residents access services, such as medical transportation, community events, tele-health doctors and COVID clinics.
I design programs and partner with community leaders/ vendors to meet the needs and desires of the property’s residents.
In additional, yearly assessments are conducted to see if current needs are being met, evaluate if new needs or barriers have stopped resident from living independently.
If new barriers or issues are presented, I work out a plan to remove or lessen barrier.
Assessments are completed and documented on ASSC online for service coordinators.

During my position, as an ES worker, my primary duty was to determine eligibility for assistance programs including food, medical, emergency relief, cash assistance and daycare.
Determining eligibility was done through informational gathering via interviews and correspondence.
Other responsibilities included but was not limited to checking daily reports, utilizing BRIDGES, submitting case reads, submitting reports timely, answering phones and having face to face interaction with clients.
