* Provide and assure that supervised employees provide services in a consumer
controlled, respectful, and helpful manner.
* Assure that documentation is at all times thorough, accurate, and current, and
assure that supervised employees maintain thorough, accurate and current
documentation at all times.
* Complete all supervisory reviews in a thorough and timely manner.
* Develop corrective action plans and follow through with identified corrective
action.
* Work collaboratively and productively with supervisor, CEO, other employees,
and outside entities encountered in day to day activities.
* Exercise initiative, identify and complete job tasks independently, exercise sound
independent judgement, and decision making in areas of responsibility.
* Recognize when there is a need to use supervisor as a resource, maintain efficient
and effective time and workload management, and meet all deadlines.
* Be responsive to supervision, performance evaluations, and
consultation/coaching by your supervisor. Be open to, actively participate in, and

*Conduct, with Consumers, a formal annual re-assessment of level of need, and review and
make needed changes to Plans of Services and Supports as required by regulation or need.
*Assist Consumers and the Medicaid eligibility staff in tracking spend down obligation/share
of cost.
*Monitor on going Medicaid Eligibility to assure basic Medicaid Waiver Eligibility
*Provide documentation to support all services coordination and resource development
decisions and actions.
*Outreach/ marketing of the Medicaid Waiver Program in the League’s contracted service area.
*Complete all required forms and paperwork in a timely and complete manner.
*Assure the privacy of Consumers, by keeping all information about consumers confidential.
*Communicate with consumers (written, verbal, sign) in their mode of communication.
*Exercise initiative, identify and complete job tasks independently, exercise independent
judgement and decision making in areas of responsibility.
*Recognize when there is a need to use supervisor as a resource, maintain efficient and
effective time and workload management, and meet all deadlines.
*Be responsive to supervision, performance evaluations, and consultation/coaching by your
supervisor. Be open to, actively participate in, and commit to the development of your work
