
Service Coordinator Supervisor with strong background in coordinating services, managing caseloads, and resolving problems or concerns efficiently. Work with service coordinators, consumers and the State of Nebraska for the AD Waiver Program.
Learn and keep up to date on all regulations, policies, and procedures applicable
to the Medicaid Waiver Program, and all League policies and procedures.
* Supervise Services Coordinators (SC) and other assigned staff.
* Follow and assure that SC s know and follow applicable Medicaid Regulations,
policies procedures, and standards specific to the Medicaid Waiver.
* Follow and assure that SC s follow League Policies and Procedures.
* Provides effective and efficient communication to supervised employees,
supervisor, and fellow employees.
* Learn, follow, and assure that supervised employees learn and follow the
League’s and the Independent Living Philosophy.
* Consistently and regularly monitor supervised staff to assure that all they are
meeting all applicable standards, and are providing the highest quality services.
* Assign workloads to supervised employees in a manner that assures fairness and
efficiency among those supervised.
* Provide and assure that supervised employees provide services in a consumer
controlled, respectful, and helpful manner.
* Assure that documentation is at all times thorough, accurate, and current, and
assure that supervised employees maintain thorough, accurate and current
documentation at all times.
* Complete all supervisory reviews in a thorough and timely manner.
* Develop corrective action plans and follow through with identified corrective
action.
* Work collaboratively and productively with supervisor, CEO, other employees,
and outside entities encountered in day to day activities.
* Exercise initiative, identify and complete job tasks independently, exercise sound
independent judgement, and decision making in areas of responsibility.
* Recognize when there is a need to use supervisor as a resource, maintain efficient
and effective time and workload management, and meet all deadlines.
* Be responsive to supervision, performance evaluations, and
consultation/coaching by your supervisor. Be open to, actively participate in, and
commit to the development of your work skills, and performance.
* Schedule and keep appointments as needed.
* Maintain a high degree of honesty and integrity in performing assigned duties
and representing the League.
* Identify supervised employees’ training needs, and work with appropriate staff to
assist supervised employees to receive needed training
* Travel as needed to perform job duties. Must provide own travel mode
* Actively communicate, promote, and apply the League’s established standards of
quality, within all areas of responsibility, including with staff, consumers,
students, consultants, and volunteers.
* Attend applicable meetings and participate in appropriate training opportunities.

*Accept referrals, assure that consumer is on Medicaid, and determine eligibility for the waiver.
*With consumers, conduct assessment of needs and develop Plan of Services and Supports.
*Determine the estimated total monthly cost of a proposed Plan of Services and Supports, and
assure that services are below CAP.
*Working with Consumers, advertise, interview, complete all provider paperwork, conduct all
background checks, and approve or disapprove providers.
*Authorize services, providers, provider rates, and amounts of service needed to meet the goals
contained in Plans of Services and Supports for Medicaid Waiver Consumers.
*Work collaboratively with Consumers to schedule, “supervise” and monitor providers’
performance, as needed.
*Contact, coordinate, and confirm the Consumers’ service provision with providers of service.
*Working with Consumers, coordinate services from all authorized providers to ensure that
Consumers’ needs and desired outcomes are met.
*Continually work with Consumers to jointly review needs, measure plan achievement, and
evaluate providers performance.
*Monitor ongoing service provision by authorized providers.
*Conduct, with Consumers, a formal annual re-assessment of level of need, and review and
make needed changes to Plans of Services and Supports as required by regulation or need.
*Assist Consumers and the Medicaid eligibility staff in tracking spend down obligation/share
of cost.
*Monitor on going Medicaid Eligibility to assure basic Medicaid Waiver Eligibility
*Provide documentation to support all services coordination and resource development
decisions and actions.
*Outreach/ marketing of the Medicaid Waiver Program in the League’s contracted service area.
*Complete all required forms and paperwork in a timely and complete manner.
*Assure the privacy of Consumers, by keeping all information about consumers confidential.
*Communicate with consumers (written, verbal, sign) in their mode of communication.
*Exercise initiative, identify and complete job tasks independently, exercise independent
judgement and decision making in areas of responsibility.
*Recognize when there is a need to use supervisor as a resource, maintain efficient and
effective time and workload management, and meet all deadlines.
*Be responsive to supervision, performance evaluations, and consultation/coaching by your
supervisor. Be open to, actively participate in, and commit to the development of your work
skills.
