As a Care Manager, the primary responsibility is linking members who have but are not defined by, Intellectual and Developmental Disabilities, with supports and resources to increase or maintain their independence. In order to achieve a person centered approach to care below are various ways this is tracked and managed.

  • Facilitate and lead Life Plan meetings with individuals, their families/guardians or advocates, and with their providers if they are participating in any programs.
  • Coordinated referrals to specialists, hospitalizations, ER visits, ancillary testing, and other enabling services for individuals.
  • Monthly follow-up with individuals, families/guardians, advocates, and providers to ensure individuals are receiving quality care whether at home or in the community. As well as actively working on goals that they are interested in through providing person-centered support.
  • Educate members, their circle of support, IDT members, and other providers regarding resources that are available to them.
  • Assist members, families/guardians, or advocates in maintaining member's benefits such as Social Security, Medicare, Medicaid, SNAP, and HEAP.
  • Assist members with problem-solving life issues whether it's related to their current medical provider, living situation or day program, or relationships. This is done in a person-centered way to encourage individuals to increase their independence.
  • Provide advocacy for and with members so that they feel they have a voice and are working towards goals they feel are important to them.
  • Maintain organizational system to ensure that all regulatory trainings, members assessments, and Life Plans are being completed to ensure compliance.
  • Provide Supervision of one direct report. This entails ensuring the Care Manager is meeting all regulatory and organizational guidelines and expectations in their role. Assist with complex cases as needed, responsible for annual performance reviews, approving expenses and monitoring his/her time sheet. This also includes advocating for them when needed with work or personal related issues or concerns. And helping them to navigate the resources that are available within the organization.
  • The RTL is also responsible for assisting new enrollees to Care Management Services with the initial steps prior to receiving HCBS Services. This includes the initial I AM Assessment and Life Plan as well as linking the member or family/advocates to community supports and services. Once the Life Plan is published the RTL will them develop a justification for services and submit the request to OPWDD. The RTL will continue to act as the Care Manager to ensure continuity of care until a replacement has been identified.
