HIV Medical Case Manager

  • Participated in SPNS Initiative: Use of Social Media to Improve Engagement, Retention, and Health Outcomes along the HIV Care Continuum, 2016-2019. Engaged HIV patients in the use of a social media intervention to increase retention and achieve viral load suppression. Gained consent and conducted study surveys every 6 months. Gathered and submitted data to team members, and reported outcomes at Grantees Meeting at HRSA.
  • Maintains a caseload of 35-45 adults living with HIV. Performs Initial Intake and Needs Assessment. Keeps paperwork and documentation up to date per Case Management Guidelines.
  • Educates clients about living with HIV, preventing further transmission, benefits and challenges of HIV care and treatment and access to resources.
  • Documents case notes daily and coordinates follow-up for seamless case management.
  • Provides training and mentorship to newly hired staff and colleagues.
  • Performs HIV tests and interprets test results, at REACCH and in community.
  • Collaborates with REACCH's multidisciplinary team to assist clients in identifying and connecting with resources for mental health treatment, substance abuse programs, domestic violence services and housing assistance.
  • Attends trainings, conferences and community meetings relevant to my position. Participates in small committees at REACCH to improve service quality and patient care.
