Using my skills and knowledge as an Occupational therapist to manage members in skilled nursing facilities, adhering to CMS guidelines for skilled services to be covered under their health plan.

Collaborate with members/families that have UHC and Humana insurances, and are receiving skilled services, such as therapy and/or Nursing in a Nursing facility. I assist with community resources as well as assisting with safe discharge planning.
