Assessment

• Interviews patient and/or family to obtain data for nursing assessment; records and reports data;
• Utilizes assessment data to formulate nursing diagnosis;
• Applies knowledge of normal parameters; identifies deviation from normal, obvious patient problems, symptoms, and/or behavioral changes; reports these to appropriate authority;

Planning

• Identifies patient care problems and establishes priorities for care based on nursing and medical diagnosis;
• Understands goals and plans of care for typical patient population;

• Contributes to revision of initial patient care plan dependent upon changing needs of patient;

Implementation

• Immunization nurse, prepared Immunizations, make sure there is enough vaccines for the day, set up equipment, maintain a clean and safety environment, be polite and maintain privacy to patients
• Making sure vials of immunizations does not expired and write down lot number on cards
• Clean up and inventory vials, and equipment, make sure section is set up for next day
• Covid-19 tester, experienced with Epic for registration, using Lumira machines to do rapid antigen, and Mesa machines to do PCR test

• Acute psychiatric hospital, prepared medication for unit, administer emergency IM or PO medication to patients and watch for side effects
• Perform 15 min checks for patients who are suicidal
• As a charge nurse: serve legal holds, like 5150, 5250, and voluntary hold, update treatment plans, do updates on charts, meet with doctors and case managers about patient care
• Give report to supervisor, manage 2-5 staff
• Do admission, do head-to-toe assessment, contraband check, take vital signs, take pictures of injuries, call doctor for medication orders, make sure patients are stable to stay in unit, make appointments with psychiatrist and nurse practitioner
• Follow up with CIWA, COW, POSS, blood glucose test, Neuro checks
• Do discharges, make sure there are doctor's orders, case management orders and send Pt with prescription medication

• Assist in admission and discharge of patients
• Take and record temperature, pulse, respiration and blood pressure of patients; Observe condition of patient and report unusual conditions to charge nurse
• Assist medical or professional nursing staff in examinations or special procedures (ex. Dressing changes, catheter insertion, and translation)
• Provide general patient care, such as bathing, feeding and turning patients; help in removal and replacement of protective comfort and therapeutic devices
• Perform clerical work (ex. Re-leave for breaks, filing of papers, maintain patient's information up to date, answer phones)
