  • Utilize profound familiarity with community services to provide comprehensive solutions for older adults and family members regarding housing, home care, meal delivery, transportation, Medicare, Social Security, Veterans benefits, medical alert systems, support groups, etc. Information and referral calls answered annually: 2,500.
  • Create individualized care plans for older adults to facilitate aging in place. Enable smooth transition to community facilities when necessary. Care plans created annually: 800.
  • Design and implement senior transportation program delivering 15K rides annually, overseeing 6 full-time employees and annual budget of $1M+.
  • Design and implement senior home repair program in collaboration with Habitat for Humanity and completed 22 projects to date for low-income seniors.
  • Facilitated close communication and liaison with advocacy groups, providers, contractors, academia, city, county and state entities, professional associations and subject matter experts within the community-care industry.
  • Provide supportive counseling sessions to older adults and their family members on a daily basis.
