Currently assigned to a 36 bed telemetry floor. Responsible for ongoing assessment of patient's continuing care needs through interviews, direct observation and chart review. Collaborating with patient's and their families to anticipate individual discharge needs. Participate and contribute to daily interdisciplinary rounds to formulate comprehensive and appropriate plans of care. Initiate the discharge planning process, obtain insurance authorizations as needed and maintain working relationships with rehab facilities, home care agencies, infusion and DME companies. Maintain constant communication with doctors, nurses, patients, families and community based resources to ensure a smooth transition to the patients next level of care. Proficiently navigating several software programs simultaneously including Outlook, Cerner, Careport and Careview. Successful at multitasking in a fast paced and constantly changing environment with the to problem solve where little standardization exits.

Contracted to Virtua Hospital, Camden. Responsible for managing home care referrals for 6 hospital floors including ICU, CCU, PCU and 3 general medical surgical floors. Referring to home care agencies in New Jersey, Delaware, and Pennsylvania. Successfully coordinated home infusion for medications, TPN and tube feeds. Obtained authorization from insurance companies when needed. Proficiently navigated multiple software programs to facilitate a paperless referral. Conscientiously documented outcomes. Consistently initiated and maintained communication with physicians, outcomes manager, home care agencies, patients and family members to facilitate smooth transitions to home care.

Responsible for the initiation of home care services to a diverse client population with a multidisciplinary team approach to care. Completed comprehensive assessments and developed individual, patient centered, care plans. Provided direct patient care and education with a focus on positive outcomes. Effectively communicated with a multidisciplinary team including physicians, physical and occupational therapists, home health aides and office personnel. Proficiently completed Oasis and ongoing clinical documentation. Effectively managed the care of a caseload of up to 40 clients from admission to discharge. Responsible for initial and continuous assessment along with the formation and implementation of care pans and monitoring of patient outcomes.
