Assist with management of organizational with strength-based projects. Serve as member of interdisciplinary team of professionals, working directly with grandparents raising grandchildren, in parent-absent households. Focus on serving families with grandchildren ages birth to five years who may be at risk for developmental delays. Screen grandchildren birth to five utilizing the ASQ (Ages and Stages Questionnaire) for developmental delays or mental health. Assists grandparents with defining family goals, Medicaid, Crisis Management, obtaining Head Start/Early Head Start, coordinating screenings and family referrals to local public agencies for treatment services, social service benefits, and/or other community resources. Knowledge of current education mandates, social- emotional curricula and initiatives at the early childhood level. Effective working relationships with school and district leaders to ensure success throughout the duration of program engagement. Facilitate support group meetings with grandparents related to health goals, and collaborate with diverse community agencies to meet family needs. Write required intake summaries, service reports, and progress notes, providing parent education support materials, working collaboratively with other members of Project Healthy Grandparent’s (PHG) interdisciplinary team to address family needs.
