Assist with management of organizational with strength-based projects. Serve as member of interdisciplinary team of professionals, working directly with grandparents raising grandchildren, in parent-absent households. Focus on serving families with grandchildren ages birth to five years who may be at risk for developmental delays. Screen grandchildren birth to five utilizing the ASQ (Ages and Stages Questionnaire) for developmental delays or mental health. Assists grandparents with defining family goals, Medicaid, Crisis Management, obtaining Head Start/Early Head Start, coordinating screenings and family referrals to local public agencies for treatment services, social service benefits, and/or other community resources. Effective working relationships with school and district leaders to ensure success throughout the duration of program engagement. Facilitate support group meetings with grandparents related to health goals, and collaborate with diverse community agencies to meet family needs. Write required intake summaries, service reports, and progress notes, providing parent education support materials, working collaboratively with other members of Project Healthy Grandparent’s (PHG) interdisciplinary team to address family needs.

Provided mental health treatment, emotional wellness, rehabilitation, crisis intervention, case management, career assessments, and support services to mentally-ill young adults. Assumed lead responsibility within the program for integrating family goals and services. Provide resources for youth, family, and community development. Assist in the curation and development of mental health support and suicide prevention activities. Managed tasks of all program staff in correlation to providing individual and group family psycho-education. Facilitated parenting training groups and workshops to educate young adults in the area of early childhood development through the utilization of TBRI materials, Circle of Security, Parenting Journey, and curriculum developed for this purpose. Provided therapeutic services to children and families with a focus on healing past trauma and building healthy attachments. Provide developmental assessments of children from birth through kindergarten age; teach parents ways to enhance their child's developmental potential cognitively. Recruit clients for child development programs through contracts with community organizations and navigating community-based resources.

Determined the precise developmental issue affecting a child through assessment, planning for an Office for People with Developmental Disabilities Services, implementation of behavior and strategic utilization to maximize a child’s progress. Reviewed and submitted all paperwork to require community resources for IDD services. Initiated and scheduled referral phone calls and meetings for any specific OPWDD/IDD Services.

Develop a data-informed strategic plan to improve disability services through the special medical preventive program. Collaborate with school officials, the Family and Multidisciplinary team to develop goals and outcomes for children referred to the Special Medical Program. Recommend evidence-based strategies and creative solutions to meet individual needs. Schedule follow up phone calls to determine referral success and maintain rapport. Provided leadership, Crisis Intervention and conduct training throughout the case plan until progression and completion of each case plan. Participate in group meetings with other Community Health Workers.
