Locations: Agrace Hospice, Watertown Reginal Medical Center – Medical-Surgical 2nd floor, Stoughton Hospital Med-Surg floor, The Marquardt Skilled Nursing Facility, East Madison Hospital Med-Surg 3rd floor

• Provide high-quality patient care to an average of 50 patients per day in a facility with 33 total beds for ambulatory surgery and procedures, all age groups
• Assess vital signs, document information in Health Link (Epic) software, report major changes to nursing staff; assist nurses and doctors
• Adhere to safety guidelines, institutional policies and company regulations
• Regular responsibilities include vital checks, running blood glucose or urine tests as needed, removal of IV, discharge patient out to vehicle.
• Prepared treatment rooms for patients by cleaning surfaces and restocking supplies.

• Provided guidance and support to individuals with various physical and developmental disabilities in their daily living activities.
• Facilitated community integration activities for clients, such as recreational outings and social events.
• Provided compassionate and personalized care, assisting with bathing, grooming, dressing, meal preparation and feeding
