Provide comprehensive case management services for the adult reentry and community program for program participants as they navigate their career pathway working in partnership , identifying strengths and challenges to achieving goals.
Work closely with other program staff to monitor progress, and coordinate referral support.

Worked with external partners (such as literacy, behavioral health and housing providers) to ensure that services are coordinated and participants are able to fully engage in career pathway services.

Provide case management services including but not limited to Individual Development Plan(IDP) development, referral for supportive services, outreach and connection to community
resources to address barriers to employment and life management challenges.

Conduct investigations of alleged child abuse or neglect to determine if and what interventions are
necessary
Assess abuse and neglect allegations reported by law enforcement, school, medical or anonymous reports

Investigated family placement options to ensure children safety
Conducted needs assessments to determine appropriate services and develop goals and care plans
Record cases information, complete accurately all necessary forms and linking and coordinating a variety of available services
Evaluate client risk and assess need for immediate intervention.

Conducted psychoeducational presentations within the community, raising awareness about mental health topics while broadening outreach efforts.
Work with families to build a child welfare system that is safe, secure, stable, and family centered which relies on evidence-based practice necessary to support contract services Provide in-home services and monitor cases beyond release from custody
Document all contact with clients and collateral contacts; complete case activity logs and all related documentation; maintain client files.
Assisted youth and family/responsible caregivers in the facilitation and coordination of the Individual Resiliency Plan (IRP) including providing skills support in the youth/family's self-articulation of personal goals and objective
Develop service and resource coordination to assist the youth and family in gaining access to necessary rehabilitative, medical, social and other services and supports
