Family Caregiver Support Program

• Meet goals and objectives outlined in the current Area Plan approved by the CA Dept. of Aging.
• Develop and plan caregiver educational trainings and support groups.
• Provide in-home and phone assessments, for the aging population
• Develop and maintain a county provided In Home Provider list that entailed accurate employment records for independent caregivers. Monitor DOJ background checks and references.
• Identify caregiver related skills and abilities, refer caregivers to employment and training opportunities.

Long Term Care Ombudsman Program

• Built a positive relationship with residents in both Skilled Nursing Facilities and Residential Care Facilities.
• Recruit and train volunteer LTC Ombudsman and, provide through case investigation reporting to California Department of Public Health and Community Care Licensing.
