Support staff of twelve service coordinators that serve a population of individual with severe persistent mental illness as well as induvial’ with a new mental health new diagnosis. Review metrics, analysis, and update processes to engage member to in behavior to help support healthy behaviors. Engage staff to learn new skills and support development. Implemented monthly trainings on NCQA processes. Leading transition specialist pilot, part of HCBS-AMH program, and Housing First Initiative. Collaborate with Optum Behavioral Health Advocates and IMAPs program.

Provide instruction for national trainings for new employees and on-going instruction. Work with leadership to update or create a curriculum to reflect changes in health plan. Utilize knowledge of several systems and state processes used in state specific trainings.

Manager to team of eleven employees located throughout Texas and Florida who were focused on positively impact Star and Hedis Rating. Navigators address barriers such as housing, clothing, and financial constraint to getting preventive care. Through phone outreach and home visits navigators work to connect member to the healthcare system. Developed new programs, policies, and processes which have now expanded into 12 states and over 70 employees.

Assisted transitioning individual who were who are in nursing home transition to independent living. Coordinated relocation process to put home and community-based services in place to ensure a safe relocation. Provided weekly support for members enrolled in the UT Behavioral Health Pilot.
