• Home Infusion/Specialty Pharmacy

- Goal is to provide exceptional patient-focus infusion care including but not limited to: IV Fluids, TPN, antibiotics, IV medications via syringe push, Specialty IV meds including antiemetics and pain management pumps

Duties include:

• Served in the Clinical Nurse Liaison role, as an Intermediary between hospital Case Management and the patient, as well as Coordination of Home Health.
• Managed the Nursing On-Call Schedule
• Coordinated Nursing visits
• Skilled in providing hands-on patient education and collaborating with the hospital care team, the patient and their family to bridge the pathway to shorter hospital stays. This includes advocating for the patient and individualizing their careplans to each specific need and Home Infusion allows the patient to receive IV therapies in the comfort of their own home.
• Mentored and trained new Clinical Nurse Liaisons in navigating complex healthcare systems, fostering professional growth among colleagues.
• Collaborated with physicians and case managers to determine optimal care settings for each patient based on clinical needs and insurance coverage.
• Increased referral volumes by building strong relationships with community partners, including hospitals, skilled nursing facilities, and home health agencies.
• Streamlined care coordination by facilitating timely admissions and discharges for patients transitioning between facilities.
• Monitored patient reactions after administering medications and IV therapies.
• Facilitated smooth transitions of care, collaborating closely with interdisciplinary teams.

• Attended high-risk deliveries, including vaginal and C-section births.
• Assisted with mechanical ventilation of premature and term infants and insertion of PICC lines and other central lines including UVC/UAC.
• Became highly skilled in managing oral feedings, gavage feedings and managing high acuity assignments.

• Quickly promoted to Charge Nurse managing a 20 bed unit. This floor eventually became an Adult Overflow Unit in addition to caring for the Pediatric population.
