Core Responsibilities:

• Client Assessment and Needs Identification:
• Assess clients to understand their needs and develop individualized care plans.

• Plan Development and Implementation:
• Developed and implement comprehensive case management plans that address clients' needs.

• Documentation and Record Keeping:
• Maintain accurate and timely case records, including assessments, progress notes, service plans, and other relevant documentation.

Responsible for, clinical assessments, development of recovery plans in collaboration with the treatment team and provision of services based on Trauma Informed and Patient Centered Model requirements, consumer need, and performance contract requirements. Coordinated links and monitored appropriate services and consumer progress with meeting recovery goals. Delivered interventions such as skills training psychosocial rehabilitative services, case management, and medication training/ monitoring and supports. in creating and managing provider service plans and participated in the evaluation of current and future needs of the client, including but not limited to substance abuse referrals, counseling and skills training.

• Performed clinical assessments to determine service needs and progress with consumer treatment goals.
• Participated in formulation of the consumer recovery plans and assures recovery plan is written and authorized with an LPHA signature for consumers on caseload.

Essential Job Functions:

• Forensic investigations of reports of abuse/neglect to children to determine if abuse or neglect occurred and conducted assesments to determine the current or future risk of harm to children.
• Observed children for signs of any harm and assess the signs to determine if they are the result of abuse or neglect, sometimes involving children with serious injuries and child fatality.
• Determined action to be taken to remove or to reduce an immediate threat to the safety of a child to include working with families to identify family members who can assist with keeping the child safe, testifying in court to seek emergency protective services, completed home studies and assessments, placed children in substitute care, referring family for immediate crisis intervention therapy or other community resources.
• Gathered family and kinship information to support the child in a placement, should the child be placed in DFPS custody.
• Developed and maintained effective working relationships with law enforcement officials, judicial officials, legal resources, medical professionals, and the community.
