-Daily care of 6-8 residents

-Taking and recording vital signs

-Encourage residents to participate in group activities and outings

-Answering call lights in a timely manner to ensure proper assistance

-Followed patient care plan regarding ambulation, feeding and individual needs

-Assist in getting patients to and from appointments, the grocery store, and any other special outings they request

-Cooking and cleaning according to their service plan

-Assistance in creating behavioral plans

-Assist in daily living needs such as cooking, cleaning, self care

-Provide companionship

-Maintain a clean, safe, and well-organized environment

-Assist with crisis intervention to de-escalate challenging behaviors

-Assist residents with daily living needs to maintain dignity and overall general wellness

-Engage residents in meaningful conversations as well as encouraging story telling and past time memories

-Assist with feeding and monitoring liquid and food intake as well as documenting appropriately

-Recognize and report abnormalities or changes to the appropriate personnel
